This 11-item scale evaluates observable signs and symptoms including resting pulse rate, sweating, restlessness, pupil size, bone or joint aches, runny nose or tearing, gastrointestinal upset, tremor, yawning, anxiety or irritability, and gooseflesh skin. Each item is weighted based on clinical significance and scored, with total scores ranging from 0 to 48 indicating withdrawal severity: 5-12 (mild), 13-24 (moderate), 25-36 (moderately severe), and 36+ (severe withdrawal). The COWS provides objective quantification of withdrawal intensity, guiding medication dosing decisions during detoxification and determining appropriate timing for buprenorphine induction to avoid precipitated withdrawal.
