The Opioid Risk Tool (ORT) is a brief, validated screening instrument that predicts the risk of opioid-related aberrant behaviors in patients being considered for long-term opioid therapy for chronic pain management. This 5-item self-report tool assesses risk factors including personal and family history of substance abuse, age, history of preadolescent sexual abuse, and psychological disease, with different scoring weights for male and female patients. The ORT generates risk categories: low risk (0-3 points), moderate risk (4-7 points), and high risk (8+ points) for developing problematic opioid use behaviors if prescribed long-term opioid therapy. Healthcare providers use this tool during the initial evaluation before starting opioid therapy to identify patients who may require closer monitoring, more frequent follow-up visits, utilization of prescription drug monitoring programs, or consideration of alternative pain management strategies. The ORT does not diagnose addiction or contraindicate opioid therapy, but rather helps clinicians implement appropriate monitoring and safeguards proportional to risk level. In the context of the opioid epidemic, systematic risk assessment using validated tools like the ORT supports safer opioid prescribing practices, helps providers make informed decisions about pain management approaches, and identifies patients who may benefit from co-prescription of naloxone, limited quantities, or integration of addiction medicine specialists into their care team.
